Your placement checklist

If you will be attending placement in the fourth year of your program, you’ll need to submit all relevant documentation to Sonia by the end of T1 of your fourth year of study.

Requirements for most health placements comprise:

  • vaccinations and health tests
  • licenses and certificates
  • online training on topics such as health and safety, inclusion and use of personalised protective equipment (PPE)
  • forms

Based on the current COVID-19 response, Mask Fit Testing is also a requirement for some students according to placement allocation.

You should give yourself plenty of time to complete the placement requirements listed below, as some steps take time to complete. Note that you'll re-use and renew several of these documents throughout your career as a health professional.

Vaccinations and Health Tests

To go on placement you may need to provide evidence of current vaccinations or immunity. Please refer to your Checks page in Sonia to confirm the vaccination requirements for your program.

For proof of vaccination you can provide your Immunisation History Statement available through Medicare. Please ensure that the Immunisation History Statement doesn't contain your Individual Health Identifier (IHI) as this is identifiable personal information. New statements do not include the IHI number so please download a new copy if required. For all other vaccinations records please ensure your name and date of birth are clearly legible so the records can be identified.

If you're not eligible for Medicare, you can get your Immunisation History Statement online through myGov. If you don't have a myGov account, it's easy to create one.
