This NHMRC research project is a implemented-effectivess trial of evidence-based care in community mental health services. Based on service provision models of CAMHS and CYMHS, clients presenting to these health services frequently experience severe and complex OCD, highlighting the need for appropriately dosed CBT-ERP commensurate with clinical need. Thus, this trial will examine the comparative effectiveness of standard CBT-ERP, versus an enhanced model of care comprising multi-modality CBT-ERP, incorporating additional parent training sessions, and extended within-session ERP as needed. As such, participating services will be quasi-randomised to one of two treatment conditions:

1. Standard CBT-ERP, delivering best-practice screening, assessment, and child-focused individual or group CBT-ERP, or
2. Strong CBT-ERP, which incorporates multiple modalities of CBT-ERP, including child focused CBT-ERP, and concurrent parent training and intensive ERP sessions (1 to 3 additional 3-hour intensive ERP sessions)

This trial is an effectiveness-implementation trial, with iterative consultation and co-design planned throughout its duration. For ease of reviewing this Type 1 hybrid implementation-effectiveness trial design – there are three streams of work, including:

1. Co-Design: Iterative co-design and consultation with clinicians, health service stakeholders, and consumers to ensure all aspects of the trial and the model of care are acceptable and feasible.
2. Clinical Effectiveness: a parallel groups RCT of standard MoC versus strong MoC implemented in QLD CYMHS and NSW CAMHs for children and young people presenting with OCD, to reduce OCD symptoms and diagnoses. Secondary outcomes include cost-effectiveness of the two interventions
3. Implementation Outcomes: evaluation of the effectiveness of our implementation strategy –  clinician training and supervision in the MoC, tailored to the health service needs and intervention conditions.
