New customers must Register with our service before submitting a request form. Current customers may also re-register to update billing details.

- Follow the sample submission requirements below.
- Enter the required information on the form and then email a copy to the GUDSF. Emailing the form assists with data entry and also in tracking your samples.
- Enclose a copy of the request form with your samples.
- Upon receipt of your samples, the GUDSF will notify you by email and inform you of the unique batch number assigned to your samples. This number will form part of the sample file name.
