Ethics Forms
To assist with the process of seeking University ethical clearance for human research, a range of proformas have been produced. Proformas have also been produced to assist teams in meeting their monitoring and other obligations.
As a result of a comprehensive review of research ethics at Griffith University during 2003, a number of these forms are currently under review. Before preparing an application for ethical clearance, always check back to this site to ensure you have the current form.

- HREC Clinical Trial Application Form - This sub-form should be completed in addition to the human project proposal form ONLY for clinical trial-based research.

Externally Approved Human Research Ethics
Human Research Ethics Approvals that have already been approved by another Ethics Committee now qualify for a special administrative review conducted within Office for Research. Applicants are asked to complete the below form and attach it to the review outcome and original application submitted to the other committee for submission to the Office for Research.

- Prior Review S17 Declaration (rtf) - The contact person signing the attached s17 declaration
- Prior Review S18 Prior Review (rtf) - Please arrange for an appropriate authorising officer, who is not a member of the research team, to complete and sign the attached s18 declaration.
